Hospital to homeHome visitComing soon

Coming Home From Hospital? We're Here for the First 7 Days.

The first week home is when readmissions happen. We own that week: clinical review within 24 hours, nurse visits, and daily care support.

About this service

Coming home from hospital should be the good part — but the first week home is when things most often go wrong. Our seven-day package owns that week: a senior clinician at your door within 24 hours, your medicines checked against the discharge letter, nurses through the week, a carer every day, and a proper handover to your GP when you're established.

Is this right for me?

  • You or a relative is being discharged and worried about coping
  • The first days at home feel daunting after a hospital stay
  • Medicines have changed and need sorting out safely
  • You want clinical eyes on recovery before your GP takes over

Important — this is not an emergency service

This is not an emergency service. If the patient has severe difficulty breathing, severe chest pain, is unconscious, has signs of a stroke (face drooping, arm weakness, slurred speech), severe bleeding or is seriously unwell, call 999 immediately.

What can we help with?

  • Senior clinician review within 24 hours of discharge
  • Medication reconciliation after hospital
  • Nursing and daily care support
  • Escalation if recovery stalls
  • Structured handover to your GP
The first few days at home can be the hardest. We provide clinical review, nursing and practical support while you regain confidence.

Frequently asked questions

What's included in the first week?

A senior clinician review within 24 hours, medication reconciliation, nursing and daily care support, escalation if needed, and a structured GP handover on day 7.

How quickly do you start?

We aim to review within 24 hours of discharge, subject to availability and location.

Can you sort out changed medicines?

Yes — medication reconciliation after a hospital stay is a core part of the service, reducing the risk of errors.

What if recovery isn't going well?

We escalate promptly — to our GP or, if needed, back to hospital.

Can I arrange this for a parent being discharged?

Yes, with the patient's consent — many families arrange this before a parent comes home.

What happens after 7 days?

We hand over to your GP, and can continue with nursing, personal care or ongoing support if you'd like.
