COPD, heart failure, dementia and palliative care
Home visitLong-term conditions
Long-term condition care at home is planned medical and nursing support for COPD, heart failure, dementia and palliative needs. We work with your NHS consultants and GP, not instead of them.

This is not an emergency service. If someone has chest pain, stroke signs, severe breathing difficulty, heavy bleeding or collapse, call 999 or go to A&E. We work alongside the NHS, not instead of it.
Long-term condition care at home is planned support for illnesses that do not fit a single appointment: COPD, heart failure, dementia, Parkinson’s, diabetes, and palliative care when comfort and dignity come first. It is for families who are already in it, not for people hunting a miracle.
We do not take over your NHS consultant, COPD nurse, heart-failure nurse or Parkinson’s nurse. We add hands and time in the house: monitoring, medicines, personal care, and a GP when control drifts. The plan should still make sense to the team that has known the person for years.
Palliative care at home is support, not a promise to replace hospice or the Trust. We will be plain about what we can do overnight, and what still belongs to 999 or specialist palliative services. Dementia care is support and safety, not a diagnosis in one private visit.
Services in this family
These are the visits we actually book. Each page says who it is for, what happens, who delivers the care, and the from-price.
COPD Flare-Up Care at Home
A five-day home treatment plan for COPD flare-ups: GP assessment, daily nurse visits, nebuliser support and a plan to catch the next one earlier.
Heart Failure Support
A week of structured stabilisation: daily home monitoring of weight, blood pressure and oxygen, nurse visits, and doctor-adjusted treatment.
Dementia Support at Home
Consistent, familiar carers and clinical oversight that help people living with dementia stay safe and settled in the home they know.
Palliative & End of Life Care
Skilled, compassionate nursing and care for people who choose to spend their final months at home — alongside your GP, district team and hospice.
Diabetes Support at Home
Regular monitoring, medication support and education at home to keep diabetes stable and complications at bay.
Parkinson's Care at Home
Coordinated nursing, therapy and care support that adapts as Parkinson's changes — keeping routines, medication timing and independence on track.
Who this is for
- COPD, heart failure, dementia, Parkinson’s or diabetes that is getting harder to manage at home
- A family arranging ongoing care for a parent, not a one-off GP slot
- Palliative needs where the person wants to stay at home and the family needs a team
- Clinic attendance has become the barrier to decent control
When this is not the right service
- A new emergency — sudden severe breathlessness, chest pain, stroke signs or collapse is 999
- A request to stop all NHS care and “go private” for a complex disease
- A one-off second opinion with no plan to involve the existing specialist team
How we arrange care
1. We start with the current NHS plan
Bring clinic letters, inhalers, insulin pens, blister packs. Guessing a long-term plan from a web form is how people get harmed.
2. A clinician sets what we will actually do
That might be nursing reviews, a GP home visit, diabetes checks, Parkinson’s timing support, or a palliative conversation. Not every condition needs every visit type.
3. We review, we do not set and forget
Long-term illness moves. If home is no longer safe, or if specialist NHS care is the honest next step, we say so.
Who delivers your care
GMC-registered GPs, registered nurses, and, where the condition needs it, physiotherapy and trained carers. Specialist dose changes for Parkinson’s or complex insulin stays with the team that owns the long-term prescribing, unless we are asked to help in a defined way.
Working alongside the NHS
Keep your NHS GP, consultants and specialist nurses. Retinal screening, pulmonary rehab, structured diabetes education and hospice pathways stay where they belong. We share readings and letters with consent. We do not hide an admission that should happen.
Where we cover
Ongoing home support covers Lurgan, Craigavon, Portadown and nearby villages, with Banbridge, Lisburn, Armagh and Dungannon arranged subject to travel and availability. Regular visits are easier to plan closer to the clinic. Call 028 9621 1981 if you are further out.
Pricing
Long-term condition programmes are coming soon and will be quoted after we understand the routine. If you need a clinician now, GP home visits start from £255. Nothing on this page is a package until we have spoken.

Common questions
Do you diagnose dementia in one visit?
No. We can do a structured review and help with the next step. A dementia diagnosis belongs in a proper pathway, not a single private appointment and a label.
Will you change my COPD or heart-failure inhalers and tablets?
A GP can review and, where appropriate, adjust or write to your NHS team. We will not rip up a specialist plan because a private visit is convenient.
Is palliative care at home the same as hospice?
No. Hospice and NHS specialist palliative care remain essential for many families. We add nursing, GP and personal care at home. We will say when hospice or hospital is the kinder answer.
Can you help with insulin at home?
We can support monitoring, timing and education, and a GP can review. Starting or stopping insulin is a clinical decision with a plan, not a web add-on.
