When heart failure flares, the difference between staying home and being admitted is often simply someone watching the numbers. For a week, we watch them daily — weight, blood pressure, oxygen — with a doctor adjusting your medicines as the numbers move, nurse visits through the week, and clear education on managing fluid and salt.

After your visit
Trends are tracked day to day so deterioration is spotted early. Treatment is adjusted by a clinician, and — with consent — your NHS GP and cardiology team are kept informed.
We provide this service across Lurgan, Craigavon and surrounding areas. Some surrounding locations may be available depending on the service, travel requirements and clinician availability.
Not sure if we cover your area? Call 028 9621 1981 with your postcode and our team will check — or see Areas we cover.
Booking for someone else?
