Many hospital admissions in older people are caused by medicines — too many, clashing, or taken wrongly. Our clinician sits down at home and goes through everything: what's prescribed, what's bought over the counter, and what's lurking at the back of the cupboard. You get a clear written outcome, and so does your GP.
Is this right for me?
You or a relative takes five or more medicines
There have been recent falls or new confusion
There have been repeat hospital admissions
You're getting side-effects or aren't sure what everything is for
Several different prescribers are involved
The family worries about 'all these tablets'
This is not an emergency service. If someone has chest pain, stroke signs, severe breathing difficulty, heavy bleeding or collapse, call 999 or go to A&E. We work alongside the NHS, not instead of it.
What can we help with?
Full medicines reconciliation — everything you take
Interactions, side-effects and adherence
Simplifying or safely reducing medicines
Dosette box and administration advice for carers
A written outcome for you and your GP
Not sure if this is needed? Call us and describe the situation — we'll advise honestly.

After your visit
You get a written outcome and, with consent, so does your NHS GP. Any medication changes are made within the clinician's scope and appropriate prescribing arrangements — nothing is changed without the right authority.
We provide this service across Lurgan, Craigavon and surrounding areas. Some surrounding locations may be available depending on the service, travel requirements and clinician availability.
Not sure if we cover your area? Call 028 9621 1981 with your postcode and our team will check — or see Areas we cover.
Booking for someone else?
Yes — a family member, carer or authorised representative can contact us to arrange this for a patient, provided the patient consents. Many of our bookings are made by a son or daughter for a parent.
Frequently asked questions
Does this include changing my medication?
The clinician reviews your medicines and makes recommendations. Any changes are made within the clinician's scope and appropriate prescribing arrangements — and communicated to you and, with consent, your GP. We never change medicines without the proper authority.
Who is it particularly useful for?
Anyone taking five or more medicines, with recent falls, new confusion, side-effects, difficulty remembering doses, or several different prescribers involved.
Do I need to prepare?
Just gather everything you take — including over-the-counter items and supplements — so nothing is missed.
Will my GP be told?
With your consent, we send a written outcome to your NHS GP so your records stay joined-up.
Can carers get advice too?
Yes — we give practical advice on dosette boxes and safe administration for family and carers.
